Anticoagulation Clinic


The Mercy Tiffin pharmacy department offers an Anticoagulation Clinic, managed by David Gardner, R.Ph. This clinic assists patients with the regulation of their “blood thinner” medication.

Most patients who need anticoagulation use a medication called warfarin, also known as Coumadin® or Jantoven®. Warfarin is considered a high risk medication by the Institute for Safe Medication Practices (ISMP). The clinic is designed to help both physicians and patients maximize anticoagulation therapy and avoid unwanted side effects from these medications.

Patients enrolled in the Anticoagulation Clinic receive care by specially trained pharmacists. During the visit the pharmacists discuss the patients’ medications, diet, recent health, recent illness, side effects, medication schedule and answer any questions or concerns.

Patients also have a small drop of blood obtained with a fingerstick that is used to check their INR (international normalized ratio). They are immediately informed of their INR and any changes to their warfarin dosage is made at that time. The clinic also provides their physician with all information from each visit and consults with him/her as needed.

This clinic is open to any patient who is taking warfarin (Coumadin® or Jantoven®) and has a referral from their physician.
