How To Get Therapy Started
If you suspect your child may need skilled therapy, contact your pediatrician for a referral. The physician can fax the prescription or you can bring the prescription to your first appointment. Our fax number is 419-251-0075.
Call your insurance provider. Find out whether your policy covers the type of therapy you are requesting, the number of visits allowed and whether your policy requries co-payment. It is your responsibility to understand your insurance benefits.
Call our office at 419-251-0070. During the phone call, you will need to provide basic information about your child such as their name, date of birth, diagnosis from doctor, insurance information, and the name of their primary doctor. Our secretary will schedule an evaluation with one of our therapists.
The evaluation will take about one hour. Many questions will be asked about your child's past medical history, birth history and current skills. Observations of play and child friendly standardized tests are usually performed to determine your child's strengths and weaknesses. The information obtained is used to determine if your child has a need for therapy.
If your child does need therapy, an appointment for treatment will be scheduled after the evaluation.
