Why is an "Interdisciplinary Team" important?
Serious, life limiting illnesses require the attention of several medical sub-specialists and health care professionals. Research has consistently shown that inter-disciplinary teams deliver the highest level of care.
Our interdisciplinary teams include physicians, nurses, chaplains, social workers and therapists.
Who can use palliative care?
Palliative Care Across the Lifespan is available to any patient with a serious life limiting illness.
How do I know if a referral to palliative care is right for my family now?
Usually, an early referral is best. Our support services can be very helpful when started soon after the diagnosis of a very serious illness. Our goal is to treat distressing symptoms and support you and your family as difficult medical care decisions are made. Our services can lessen worry and confusion often felt during this difficult time.
Does this mean we have to give up hope?
No, not at all. We will hope with your family while we support you as you make tough medical decisions. Medical care to treat your illness can continue alongside palliative care. We will provide relief of symptoms, help you make the most of every day and, if necessary, help prepare for a peaceful death. We never give up hope!
