Insurance and Billing

Mercy Laboratories accepts many insurance plans.

Please send insurance and demographic information with the laboratory specimen. Information necessary to submit a claim includes: full name, date of birth, current address, telephone number, insurance carrier, and subscriber ID number. If patient is under 18 years old, please provide the necessary demographic information for the parent or legal guardian.

After insurance payment has been received, we will bill any balance to the patient. Generally patients will not receive a bill until insurance has responded with a payment or denial. If an EOB has not been received from the insurance carrier within 45 days of filing, we may bill the patient and ask them to contact their insurance carrier.

Courier Service

Mercy Laboratories provides daily pickup and delivery service Monday through Saturday. Our dedicated lab couriers are focused on your laboratory service needs.

Courier service is available for transporting specimens, reports & supplies to all locations in greater Toledo, Bowling Green, Genoa, Elmore, Lambertville and Temperance, Michigan areas. Regular or STAT pickups may be arranged by calling dispatch at 419.251.8260. Lock boxes are provided when late speciman pickup is requested.

A call is appreciated whenever your office hours are changed, we want to work closely with you to avoid any missed or delayed specimen pickups.

Supplies

Supplies necessary for the collection, preparation and preservation of specimens submitted for testing are provided free of charge to our clients. Incidental supplies are not permitted. In compliance with the established Federal Guidelines, we may provide only those supplies specific to the testing being ordered.

We also provide pre-printed custom requisitions for clinical laboratory, cytology and surgical pathology free of charge. Supply requisition forms can be completed and sent along with the courier or faxed to 419. 251.7904.
