Outpatient Diabetes Education* Program

Helping you achieve better managed diabetes
 

The Outpatient Diabetes Education* Program empowers patients to successfully manage their diabetes by teaching the skills needed to keep blood glucose levels in target ranges. Individualized plans are based on the American Diabetes Association's (ADA) standards of care. Our staff includes two Certified Diabetes Association Educators.

One-to-one and group education sessions are available. Classes focus on general disease overview, medication information, nutrition and meal planning, exercise, blood glucose monitoring and goal setting. Our supportive team will provide management skills and motivational strategies to promote healthy life changes and prevent complications associated with diabetes.
