Bring Your Insurance Cards to Every Appointment

Please present your current insurance card or cards at Central Check-in when you arrive for your appointment. Our staff will verify that we have correct information for you.

Co-pays

If you have an insurance co-payment, it will be collected at Central Check-in.

Automatic Filing

You can arrange with the staff at Central Check-in for automatic filing of primary and secondary insurance.

Disability Forms

Please complete your section of the form and bring it to the Insurance Office. The staff will see that the physician's part is completed.
