Central Check-In

Established Patients

Patients need to stop at Central Check-in before each appointment. Insurance co-pays will be collected and account information will be verified at Central Check-in. Patients are asked to bring their insurance cards.

New Patients

New patients need to arrive about 15 minutes before their scheduled appointments and go to Registration, which is located in the West (Pharmacy) Lobby. New patients should bring:

• Driver's license or other photo ID
• Insurance cards
• Any legal documents that might be required, for example, a signed parental consent to treat, if the person bringing the child to the appointment is not the parent or guardian. (Click here for a blank consent-to-treat form.)
