Mercy Family Care

Roles & Responsibilities

The Mercy Family Care team:

Pam Oatis, M.D. - Medical Director
  • Consult on critically or complexly ill patients in prenatal, neonatal, or pediatric age range for inpatients and out patients to address symptom management, goals of care and development of a personalized care plan focusing on quality of life
  • Provide and teach Listening with Connection, a listening focused communication tool based in Re-evaluation Counseling
  • Project director for grants funding team services
Joyce Roe, RN - Medical Home Liaison
  • Assist and educate the family on how to use their Primary care office to meet the needs of the child and the family.
  • Attend physician rounds to help reinforce tests and treatments that are being done and to be a resource if the family has any questions.
  • Communicate with Primary care offices while their patient is in Mercy Children’s Hospital by giving weekly updates.
  • Attend Care conferences
  • Refer to Medical Legal Partnerships if needed
  • Assist families to in building of a Medical Care Notebook
  • Participate at time of discharge to coordinate appointments and coordination of care.
  • Follow up with families after discharged to make sure they are well connected with their PCP and their needs are being met.
  • Provide Family Voices, Bright Futures and American Academy of Pediatric resources.
  • Educate potential offices about the Medical Home Project.
  • Practice, provide and teach Listening with Connection to families
Angie Westenkirchner, RN - Maternal Fetal Navigator
  • Navigate the care of and provide resource to women whose fetus has been found to have a life threatening or life limiting condition.
  • Provide support throughout the pregnancy and delivery and help connect them to area resources
  • If the baby’s condition is lethal, help the family develop an after delivery plan that is meaningful. Distribute that plan to all concerned well ahead of the delivery.
  • With the family, explore funeral homes, hospice and other services that they may want to consider before the birth of their child.
  • Practice, provide and teach Listening with Connection to families
Sr. Maxine Young - Pastoral Care Chaplain
  • Provide spiritual support to pt, families & staff.
  • Provide baptism and blessing or any other request from the family.
  • Along with nurse provide a grief packet to parents whose child has died.
  • Offer to call family’s church and/or Pastor
  • Offer presence and encourage listening and sharing of feelings as needed.
  • Support family goals for care and an advocate for the family
  • Provide support and coordinate care with staff
  • Organize the volunteers who put together the Grief packets(knitted hat, gown & blanket)
Sr. Cecile Sakaley - Family & Mission Services
  • Support parents/families while their child is in NICU or PEDS.
  • Give individual love and attention as a cuddler when parents are unable to be present.
  • Provide tutoring for school-age patients.
  • Provide Guided Imagery & Hand massage for pain management or anxiety.
  • Engage long-term ante partum and PEDS patients/families with Scrapbooking to record their hospitalization.
  • Escort in patients to MFM for ultrasound and make referrals to Palliative Care when appropriate
Estil Canterbury - MA, PC-CR and Listening with Connection Specialist/Counselor
  • Listening with Connection (LWC) counselor for anticipatory grief, bereavement, hurt, confusion, loneliness, fear, personal development, and medical crisis counseling.
  • Student of LWC.
  • Provider of child-centered play therapy for children too young for LWC.
  • Advocate for parents.
  • Provider of continual contact with parents via inspirational text messages, bereavement/Thinking of You cards, and invitations to attend “A Time for You”.
  • Facilitator of “A Time for You” support group for current families, discharged families, and staff whereby homemade bread and soup is served, and LWC is taught and practiced every Friday from noon-one in MOB 2, Ste.1300 conference room. Look for signs.
  • Provider of education and support to families & staff when needed.
  • Cuddler: giving individual love and attention to babies and children when needed.
  • Provider of mental health diagnosis and treatment.
  • Creator of Power Point Presentations for Foundation and other organizational talks
Meliss Klorer,  MRC - Project Coordinator
  • Oversees the grant and works with the Program Director and other partners and collaborators to implement, evaluate and alter the program as appropriate.
  • Works in collaboration with the Project Director and staff to ensure operational success and achievement of project goals and objectives.
  • Works throughout the facility and State when conducting workshops and in-services
  • Assumes the leadership role in orientation and coordination of statewide initiatives.
  • Integrates current research and evidence based practice into educational activities.
Nicole Cassidy - Family Partnership Liason
  • Responsible for strengthening the Ohio Family Voices website and social networking for information of interest to families with CSHCN, YSHCN and their medical home and family support service providers.
  • Medical Home and Listening with Connection Trainer.
  • Assists Palliative Care team in working with our patients and families with Listening With Connection.
  • Assists in putting together the Care Notebooks for families.
Tammy McKittrick…Data Entry & Office Coordinator
  • Data Entry for all patients who have received a Palliative Care Consult
  • Ensure data is submitted for grants in a timely manner
  • Team player to ensure hospitality baskets, quilts and Care Notebooks are given to the patients & families.
  • Contact with families post d/c for satisfaction surveys
  • Note taker for team meeting, Advisory Board Hospital to Medical Home meetings and MaCPac
